How to provide feedback or make a complaint

We encourage you to use our Feedback & Complaints Form wherever possible, as it helps us understand and respond to your concerns as quickly as possible.
Anonymous feedback
You are welcome to provide feedback or make a complaint anonymously. You can do this by posting a completed Feedback & Complaints Form to our postal address or by asking an advocate, family member, or another trusted person to contact us on your behalf.
If you are not satisfiedIf you are not comfortable raising your concerns with us, or if you are unhappy with the outcome, you can make a complaint directly to the NDIS Quality and Safeguards Commission.
The Commission accepts complaints about NDIS services or supports that:

were not provided in a safe and respectful way; or

were not delivered to an appropriate standard.
